There is an error message when I try to access to Hannidate release form

There is only an error message when I try to access the Hannidate release form. I emailed the webmaster with no response back. I called the number but the lady said I should contact the webmaster who isn't returning emails. I wrote to hannidate@hannity.com but my emails come back undelivered. Would someone mind sending me the release form? Thanks.
